Goal: the goal of this assignment is to learn input in Java.

Assignment:
Write a piece of code that asks the user to input his or her name, the name of the city where he or she was born, and the birthdate, and then prints the following message: 

Dear (name goes here),

Thank you for completing the survey. 

According to our data, you were born on 
(date goes here) in (city goes here).
Example:
For example, if your name is Blaise Pascal, and you were born on June 19, 1623 in Clermont, France, then your code should print the following message: 
Dear Blaise Pascal, 

Thank you for completing the survey. 

According to our data, you were born on 
June 19, 1623 in Clermont, France.
